1. On a prospect or location, select Log visit.
2. Select Add a site survey.
3. Fill in what you checked. Every field is optional.

4. Select Save visit.
• Stage change: recording a survey moves a New or Contacted prospect to Site survey.
• After conversion: a prospect's surveys move to the new location when you convert it.
